Eligibility Review

  While filing weekly claims for benefits, you will periodically be scheduled to report for an Eligibility Review.  If the following message is printed on your claim, it is a notification for you to report to your local office on the day and time designated:

ANSWER ALL QUESTIONS AND BRING CLAIM FORM TO YOUR LOCAL OFFICE FOR AN IN-PERSON INTERVIEW ON (DATE) AT (TIME).  DO NOT SEPARATE OR MAIL.

The mail claim attached to your check will have "ELIGIBILITY REVIEW" printed in the space where your name is usually written.  Bring the entire claim with you when you report to your local office.  DO NOT MAIL THIS CLAIM, as it cannot be processed for payment until you report to your local office.

 

Complete the Eligibility Review Questions before you report to the local office for your interview.  However, do not sign the mail claim until instructed to by the claims office interviewer.
